request to be contacted by\u00a0Velox Insurance and its agents, service providers, assigns and successors, (\u201cVelox Insurance\u201d \u201cus\u201d or \u201cwe\u201d) to discuss your insurance needs and the products offered by\u00a0Velox Insurance. We need your consent <span class=\"white-space\">(\u201cE-Consent\u201d)<\/span> to using and accepting electronic signatures (\u201cElectronic Signatures\u201d) and electronic disclosures and consent (\u201cElectronic Records\u201d) that authorize us to contact you for that purpose. This form notifies you of your rights when receiving and signing Electronic Records.<\/p>\n<p><strong>Option for Paper or\u00a0<span class=\"white-space\">Non-Electronic<\/span>\u00a0Records<\/strong><\/p>\n<p>You may request any Electronic Records in paper by calling us at\u00a0<span class=\"white-space\">770-293-0623<\/span>\u00a0or mailing us at 7711 Center Ave #200, Huntington Beach, CA 92647. We will provide paper copies at no charge\u00a0Velox Insurance will retain copies of the Electronic Records as applicable law requires.<\/p>\n<p><strong>Scope of Consent<\/strong><\/p>\n<p>This\u00a0E-Consent\u00a0applies only to your agreement to receive promotional and marketing calls and text messages from us.<\/p>\n<p><strong>Consenting to Do Business Electronically<\/strong><\/p>\n<p>Before you decide to do business electronically with\u00a0Velox Insurance, you should consider whether you have the required hardware and software capabilities described below. If you do not have the hardware and software required to access and retain the Electronic Records, or if you do not agree to the use of Electronic Records and Electronic Signatures, you will not be able to receive these promotional or marketing calls or text messages from\u00a0Velox Insurance.<\/p>\n<p><strong>Hardware and Software Requirements<\/strong><\/p>\n<p>To access, electronically sign and retain the Electronic Records, you will need to use the following computer software and hardware: a PC or MAC compatible computer or other device capable of accessing the Internet and an Internet Browser software program that supports at least 128 bit encryption, such as Microsoft\u00ae Internet Explorer, Netscape \u00ae or Mozilla Firefox\u00ae. If these requirements change while you are maintaining an active relationship with us, and the change creates a material risk that you may not be able to receive Electronic Records, we will notify you of these changes. You will need a printer or a\u00a0long-term\u00a0storage device, such as your computer\u2019s disk drive, to retain a copy of the Electronic Records for future reference. Please save and print a copy of this\u00a0<span class=\"white-space\">E-Consent<\/span>\u00a0to confirm that you have the required hardware and software to conduct electronic transactions with us.<\/p>\n<p><strong>Withdrawing Consent<\/strong><\/p>\n<p>You are free to withdraw this\u00a0<span class=\"white-space\">E-Consent<\/span><br \/>\n \u00a0at any time and at no charge, but doing so will not apply to any Electronic Signature that you provide us before withdrawing this\u00a0E-Consent.
